monetize youtube videos

I want to start uploading again and monetizing my videos. Other than the EA permission form found on their FAQ site, is there anything else I need to ask permission for before monetizing my game play and uploading it?

No you should be good. I didn't get any claims for any game for posting gameplay including this one.
